
When Young People Move In
Here is a quick checklist of things to do when a new young person is placed with us. This list has been broken down into multiple sections to make it easier to follow.
BEFORE THE YOUNG PERSON ARRIVES

Prepare the Placement – Ensure all available rooms and the communal areas are clean and presentable.

Read over the referral so you have a basic understanding of the young person and their needs and can action any key parts of risk management e.g. removing sharps or breakables (mirrors and screws from their room)

Prepare Young Person’s Moving in Pack – (Consisting of: 1 Set of bedsheets[2 pillows, A duvet, duvet cover, fitted sheet, 2 pillow cases, mattress protector, pillow protectors], Toiletries[toothbrush, flannel, towel, shower gel, hand sanitiser, moisturiser, face mask], Clothes airer, coloured cutlery set,

Prepare Entertainment items: Ensure a working TV is in the room set up fire tv stick, login to Netflix, Disney Plus, prime,

Take inventory of Infinity Alliance items in the room

Prepare for pre-placement meeting: Print Tenancy agreement and behavioural contract, Image consent form, YP handbook, YP folder(18+ Care Pack, independent skills assessment etc.)
WHEN THE YOUNG PERSON ARRIVES:

Assist young person with moving in, show them to their room (if there is more than 1 room available, let them choose)

Give them their Moving in Pack (see above)(Update plates register with which colour they have been assigned)

Show them around the house and ensure they know where everything is
-
Help them to make their bed,

Take an inventory of their things
-
Order their Takeout meal (let them choose what they want £20 budget)

Give them an hour to themselves so that they can settle in
-
Go Over health and safety: Fire Safety Questions, Fire evacuation point, House rules

Give them their handbook and YP Folder
-
Where possible, take young person around the area so they can be familiar with where they are (consult care plan and safety assessment before doing so)(Does not need to be completed on 1st day but does need to be done within 1st week

Collect essential information from the young person such as their contact number **

Create their Folder on the Teams system (from template)
-
Fill arrival date in chronology
-
Update front sheet with basic information (name, DOB, contact info), EDT number and social worker information

Introduce to other young people in the house
-
At end of 1st day Send a brief update of how the young person has done since moving in , how they have settled , what you have learned or discovered , things they were given, potential remarks they said.
IN THE 1ST WEEK OF PLACEMENT

Hold Pre-placement Meeting ensuring contracts are signed and young person is aware of the rules

Set curfew with social worker

Check with Social worker weekly allowance arrangements (sent directly to young people or provided by staff)

Initial shop with young person when necessary

Fill in the young person’s Front sheet
-
Contact social worker to request their previously registered doctors dentists and opticians and verify the last time they visited each. Ask if they need to be registered at local ones of each.

Set care Plan and Timetable for key working

Introduce to other young people in the house (If not done on 1st day
-
Where possible, take young person around the area so they can be familiar with where they are (consult care plan and safety assessment before doing so)(Does not need to be completed on 1st day but does need to be done within 1st week

When Young People Move Out
Here is a quick checklist of things to do when young people are leaving the placement. This list has been broken down by area
Bedroom

Clear the bedroom of old things (duvet, pillows, Sheets) - let Sam Foster know about it.

Investigate the bedroom for damages - report and book repairs.

Investigate the bedroom furniture and technology and see if there is anything missing or broken. (TV and Fire TV stick, fridge, wardrobe and drawers)- report this to be inventoried or charged for and contact management to order new ones.

Check the inventory, highlight items missing or damaged, contact Sam or management team for items that need to be replaced

Clean bedroom thoroughly

Purchase whatever is necessary to make it habitable again such as bedsheets, pillows, air fresheners etc. Consult Inventory
Bathroom
-
Clear young person’s products such as flannel, towel, shower gel etc from draws
Kitchen
-
Clean young person kitchen cupboards and Ask them if they wish to collect/keep anything left behind, if not donate it to other young people, the staff or throw away

Check their dishes (are they in a reusable condition, do we need to purchase new ones)
Documents
-
Archive all forms and documents and folders for young people on the computer or unit hard drive.

Go through a final feedback form with young person
